We take your privacy very seriously…

Melbourne Shoulder Group collect health information related to how your shoulder and/or neck feels (pain, range of motion, strength, overall function) to monitor and improve the quality of your care. We do this with a detailed physical assessment of your shoulder and/or neck in the clinic as well as using online questionnaires.

Melbourne Shoulder Group use Microsoft 365 Dynamics Customer Voice (a feature of Microsoft Office 365) for the collection of online questionnaires as well as some of your personal information (name, email, date of birth) so that we can allocate your answers to your clinical profile.   This information is stored securely in the United States under the European Union (EU) General Data Protection Regulation (GDPR). Your personal and shoulder information is always securely stored, and your identity is always kept confidential and is never revealed or published. We only use your personal information for standard care practises (for example, contacting your GP about your condition) or if we need to contact you about this questionnaire, and never for marketing purposes. Microsoft 365 Dynamics Customer Voice does not sell your personal information to third parties.

Your online questionnaires are primarily used to enable your treating practitioner to provide informed treatment to you as well as monitor your progress.

The Melbourne Shoulder Group also undertake clinical research in an effort to study and improve the treatment of neck and shoulder conditions.

With your consent we may use your de-identified questionnaire data for research projects. Your personal and questionnaire information is always securely stored, and your identity is always kept confidential and is never revealed or published.  Ramsay Health and Human and Research Ethics committee has approved the collection of this data for research purposes (Project No. 165).  Please note that if you are under 18 years of age, a guardian will also have to provide consent in addition to your own. If you do not wish to have information about your condition used for research projects, your quality of care will not be compromised in any way.
